Share Dealing Forms

1. Account Upgrade Form (PDF, 175KB)

You want to upgrade from a Certificated Account to a Nominee Account.

You want to trade in International stocks (If you want to trade in US stocks & shares you will also need to complete the W-8BEN form).

You want to trade in warrants and securitised derivatives (including Covered Warrants).

2. Single to Joint Application Form (PDF, 149KB)

You want to change your Account to a Joint Account.

3. Crest Transfer Form (PDF, 639KB)

You want to transfer your paper share certificates into your Direct Trader Share Dealing Account.

4. Asset Transfer Form (PDF, 157KB)

You want to transfer assets from other brokers (including fund management and life companies) into a new or existing Direct Trader Share Dealing Account.

5. Book Cost Form (PDF, 12KB)

You want to add or amend your own bookcosts. You can also do this online; just log in to your Account, then go to Account Information/Portfolio Holdings and follow the instructions on the page.

6. Third Party Authorisation Form (PDF, 96KB)

You want to authorise one or more individuals to act on your behalf.

7. ISA Transfer Form (PDF, 223KB) (Existing customers only)

You want to transfer your ISA (or ISAs) held elsewhere to your existing Direct Trader Self-Select ISA.

8. ISA Transfer Form (PDF, 223KB) (New customers only)

You're new to Direct Trader and want to transfer your ISA (or ISAs) to a Direct Trader Self-Select ISA

9. Bed & ISA Instruction Form (PDF, 99KB)

You want to put shares you already hold into your ISA. To do this, your shares must first be sold and then repurchased within your Direct Trader Self-Select ISA.

You will also need a CREST Transfer Form (PDF, 639KB)

10. Save As You Earn (SAYE) Transfer Form (PDF, 97KB)

You want to transfer shares from a maturing, approved employee share scheme to your Direct Trader Self-Select ISA.

Other Forms

11. Direct Trader Overseas Resident Share Dealing Application Form

You want to apply for a Direct Trader Share Dealing account but are not resident in the UK. As well as completing the Direct Trader Overseas Resident Application Form you will also need to complete either the Verification of Identity (EU) form or the Verification of Identity (non-EU) form. These along with documentation to confirm your name and address (such as passport and utility bill) need to be endorsed by a regulated intermediary (such as a bank, financial advisor, Accountant, Solicitor or Notary Public).

You want to set up an Investment Club amongst friends or colleagues.
